We are looking for a Patient Engagement Specialist to join our Chronic Care Management (CCM) team within our Product Management division. Your job will be to bring Care Management services to patients with high-risk chronic diseases on behalf of athenahealth’s customers by raising awareness and educating patients about the CCM Program. You are ready to jump in to help others on your team and collaborate to drive results. **Responsibilities**: - Educate eligible patients on the value of the program, their qualification and how the program works.
