BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a per diem opportunity performing home health visits in Columbia, South Carolina. This office services adult and geriatric clients on a per-visit basis throughout Richland and Lexington Counties. One year of clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing, helping clients remain at home. Performing diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments. Implementing protective interventions to prevent wounds and promote healing using evidence-based dressings and topical agents. Developing and updating individualized care plans including manual exercises, gait training, and balance interventions. Assessing and revising care plans regularly and collaborating with nursing staff as needed. Educating clients and families on rehabilitative activities to promote health and independence. Documenting observations, interventions, and evaluations accurately using a tablet device. Qualifications: Current South Carolina Physical Therapist (PT) license. Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills.
